Planning for Your Appointment
Provide the representative with the following information:
- Name (please be sure to spell out your name to the representative to ensure accuracy)
- Address
- Phone Number
- Date of Birth
- Gender
- Emergency Contact
- Insurance Plan, ID #, Policy holder and policy holder DOB (card will be scanned at time of your visit)
- Employer
- Identification card (e.g. driver’s license) will be scanned at the time of your visit
- Guarantor of minor (if applicable)
- Power of Attorney (if applicable)
Contact your insurance plan to determine your benefits. Your insurance is a contract between your insurer and you. It is your responsibility to know and understand the terms, guidelines and limitations of your plan. It is also your responsibility to advise us of any changes in your insurance, your address or your employer. Please reference our common plans that we participate with including our Tax ID # that you can provide to your insurance plan to determine provider participation status. We also list an area of common procedure codes that you can inquire on to your insurance company for coverage and/or out of pocket expense.
Please consult your insurance plan to determine their reimbursements and work with your primary care physician to ensure requirements are met. If you determine a referral or authorization is required per your insurance policy, we recommend you bring it with you to your appointment or confirm upon arrival that we have received. Managed care plans such as health maintenance organizations (HMOs) and preferred provider organizations (PPOs) may deny or reduce benefits if care is obtained outside of the established network or authorization of the plan.
Refer to our new patient packet to complete paperwork in advance of your visit. Please arrive 15 minutes early for your appointment and bring a method of payment to pay your copay, co-insurance and any deductible at the time of your visit.
